What Should an NDIS Invoice Include? The Complete Checklist for Support Workers
8 min read · Updated 15 October 2026 · Australia
If you've ever had an invoice bounced back by a Plan Manager, you know it's not just annoying — it delays your pay. Most rejections come down to a handful of missing or mismatched details. This guide covers what an NDIS invoice should include, line by line, so you get paid the first time.
Important: NDIS pricing and rules change. Figures and rules in this guide explain the structure — always verify current rates against the NDIS Pricing Schedule before invoicing. Rayvice is independent software and is not endorsed by the NDIS or NDIA.
Key takeaways: the 4-point checklist
- Who you are: business name, ABN (matching ABN Lookup) and contact details, plus a unique invoice number and invoice date.
- Who you supported: the participant's full name and 9-digit participant reference number (NDIS number), not a parent's or the Plan Manager's name.
- What you delivered: date, start and end times, the correct NDIS support item number, hours, rate and line total for each rate type.
- How to pay you: BSB, account number and due date. Add GST only if it applies.
Who needs to follow these rules?
Everyone who invoices for NDIS supports, whether you're a registered provider or an unregistered sole trader. Most independent support workers invoice Plan Managers, so this guide is written with them in mind. Always check the latest rules on the NDIS website, as requirements and price limits are updated regularly.
- Plan-managed: you send the invoice to the participant's Plan Manager, who checks it and pays you.
- Self-managed: you invoice the participant (or their nominee), and they pay you.
- NDIA-managed: providers claim through the NDIS portal. Unregistered providers generally can't be paid for NDIA-managed participants.
1. Provider details
- Business name: use the name linked to your ABN.
- ABN: it must match what appears on ABN Lookup. A typo here is one of the easiest ways to get an invoice rejected.
- Contact details: email and phone.
- Invoice number: it must be unique, and you shouldn't reuse a number across different participants.
- Invoice date: this should fall on or after the dates of service. You can't invoice for support you haven't delivered yet.
- Title: "Invoice", or "Tax Invoice" if GST applies.
2. Participant details
- Full name: the person who holds the NDIS plan. Don't use a parent's, carer's or Plan Manager's name, even if the participant is under 18.
- Participant reference number (NDIS number): 9 digits. A single wrong digit is enough for a mismatch.
3. Service details
That last point matters most for support workers. NDIS pricing differs by time of day, so one shift can contain several rates. A Tuesday 6 PM to 10 PM shift, for example, is two hours at the weekday daytime rate and two hours at the weekday evening rate (evening starts at 8:00 PM). Weekends, public holidays and provider travel are also billed on their own lines.
- Date of support (or date range).
- Start and end times.
- Hours or quantity delivered.
- Description of the support in plain language.
- Separate lines for each rate type.
4. NDIS support item numbers (Support Catalogue)
Each line needs the correct support item number from the NDIS Support Catalogue. It is a code like 01_011_0107_1_1 (Assistance with Self-Care Activities, Standard, Weekday Daytime). The code encodes the support category, the type of support and the time-of-day rate, so the evening, Saturday, Sunday and public holiday versions all have different codes. Two points to keep in mind: the item must match what is in the participant’s plan, and the rate you charge must not exceed the price limit for that item in the current NDIS Pricing Schedule. Provider travel and kilometre charges have their own rules — check the current Pricing Schedule for what you can claim, and show travel on a separate line.
5. Payment and banking details
- BSB and account number.
- Account name.
- Due date (for example, 14 days from the invoice date, or whatever you have agreed).
- Total amount payable.
6. GST (only if it applies)
Most disability supports are GST-free, but not all. If you're registered for GST and a supply is taxable, the document must be a tax invoice showing the GST amount. If you're unsure, check with the ATO or your accountant.
The cost of missing details
A rejected invoice isn't just paperwork — it's your income sitting in limbo. Plan Managers have to match each invoice against the participant's plan and the price limits, so these are the problems that usually stop an invoice. Each bounce can add days or weeks to your payment cycle. For a sole trader covering fuel, phone and rent, that adds up.
- Wrong or missing reference number: the invoice can't be matched to a plan and is returned.
- ABN typo or mismatch: the Plan Manager can't verify you and holds payment.
- Missing support item number: delays while they query which support you delivered.
- Rate above the price limit: the invoice is rejected or reduced.
- Evening, weekend or public holiday hours lumped into one line: rates can't be verified, so it's sent back.
- Duplicate invoice number: flagged as a possible duplicate.
- Invoice dated before the service: treated as invalid.
Example anatomy of a well-formed NDIS invoice
The figures below are illustrative only. Always apply the current price limits and your own agreed rates. Provider: example support services business, ABN 00 000 000 000, invoice INV-0042 dated 14 July 2026, due 28 July 2026. Participant: example participant, reference number 4XXXXXXXX, billed to the example plan manager. Verify every item number against the current Support Catalogue before you send, as codes and rates change between pricing releases.

Stop typing item numbers and rate splits by hand
Building this invoice in Excel means writing the formulas, splitting the hours, looking up the price caps and attaching the PDF every single time. - Log a shift in about 30 seconds: date, times, participant, support category.
- Rates split automatically into day, evening, Saturday, Sunday, public holiday and travel.
- Pre-send checks against current NDIA price caps, your ABN and BSB format, and 9-digit NDIS numbers, before the invoice is created.
- A tax-invoice PDF emailed straight to the plan manager, with delivery status you can see.
Common questions
What should an NDIS invoice include?
An NDIS invoice should include your business name, ABN and contact details, a unique invoice number and date, the participant's full name and NDIS number, and a line for each support with the date, times, NDIS support item number, hours, rate and total. Add your bank details and due date, and show GST only if it applies.
Do I need an ABN and an NDIS number on every invoice?
Yes. Your ABN identifies you as the supplier, and it should match ABN Lookup. The participant's 9-digit NDIS number links the invoice to their plan. Missing or incorrect numbers are among the most common reasons Plan Managers return invoices.
Do unregistered NDIS providers need the same invoice details?
Yes. Registered and unregistered providers use the same core invoice details. The main difference is who can pay you: unregistered providers are generally paid by self-managed or plan-managed participants, not NDIA-managed ones.
Do I charge GST on NDIS support invoices?
Most disability supports are GST-free, but it depends on the support and your GST registration. If you're registered and a supply is taxable, issue a tax invoice that shows the GST amount. If you're unsure, check with the ATO or your accountant.
